What to do when you fail MRCP?
Passing any of the three parts of MRCP is one of the best feelings in the world, and failing makes you relentlessly doubt yourself as an aspiring physician. It’s a difficult time being a core medical trainee – you’re expected to overcome this notoriously challenging diploma by February of CT2, prior to ST3 applications. This is in addition to eportfolio, procedural competencies and often times an unreasonable rota.
When I fail at something important, which is more often that not(!), I like to approach from different angles and try to the best of my ability to optimise everything. The key phrase is “best of your ability” because we’re all human and have blips here and there. Here are some thought processes:
- Are you doing everything the standard way? For MRCP Part 1, it’s well known that the “proper way” to study is to whizz through many questions using active revision. Initially you’re expected to score pretty low, unless you were an excellent medical student, but that’s what you want. The questions you answer incorrectly should prompt you to read guidelines and follow any links the sites recommend. The questions you answer correctly are often times just a waste of time if you think about it. Check out this guide for more info. For Part 2, you want this book and this one, as you want to expose yourself to as much data interpretation as possible. Minimising the time between Part 1 and Part 2 will also help. For MRCP PACES, make sure you’re taking every learning opportunity which means an acute medical rotation should be seen as an advantage. What other job will allow you to see 8 – 10 new patients every single shift and present to the registrar or consultant? Check out this guide for more info.
- If you’re studying the standard recommended way, then you should reflect on the effectiveness. You’ll need to identify a sticking point – are you spending 3 hours clerking a patient because you’re asking too many irrelevant questions? Or perhaps you need to ingrain the neurological examination into your soul so that you can actually think about your presentation and differential diagnosis whilst going through the motions. Are you mindlessly doing hundreds of questions but not reading up on the ones you got wrong? You’re really strong in cardiology and diabetes but perhaps you know nothing about haematology? It’s all about taking ownership of the problem – doing the same thing and expecting different results is the definition of insanity and all that.
- Apart from under-revising, could you be over-revising? Because MRCP is such a mountain to climb it’s not uncommon to see CMTs devote entire months to work and revision, with no other life activities. Although work-life balance is really a myth, it’s important to recognise when you’re neglecting health and relationships. Have your alcohol, coffee and treats but also make sure you have enough sleep, exercise, water and nutritious food.
The key to success after failure is to immediately apply for the next diet because you don’t want all your current revision to be “forgotten”.
You want to change things up for two main reasons. Firstly, there must’ve been a reason why your first attempt was unsuccessful. Even though PACES contains a bit of luck it’s better to err on the side of caution and just take ownership for the loss. The second reason is psychological: if you enter the exam a second time not knowing what your mistake was and having changed nothing from last time, you’re in a dark forest. You have no idea where you stand and where you’re going.
Getting an extra hours of sleep, working out three times a week and aiming for 2.5L of water per day will do wonders for you physically and more importantly psychologically.
