Are Physicians the Least Fit?

Originally I wanted to title this post “Why are physicians the least fit?” but that’s perhaps a tad self-deprecating, and not to mention I could certainly think of more than a few very fit physicians immediately off the top of my head.

So I recently dismantled my old power rack from Glasgow which I bought almost a decade ago, moved it to Dundee and reassembled it in the garage of the new house. I’ve also bought this treadmill which I might write a review of in the near future. One of the most compelling reasons to build a home gym, if you’re someone who doesn’t like to run outside, is that it removes another excuse for exercising.

Instead of locating your gym clothes and donning them followed by traveling to the gym to exercise, you can literally run barefoot on your home treadmill saving significant amounts of time and fuel. Certain exercises such as the heavy squats, deadlifts and bench press are only possible in the gym or with a sensible home set up.

In my humble opinion, for what it’s worth, physicians as a whole group are probably the least physically fit group of doctors for a multitude of reasons. Being a internal medicine or higher medical specialist trainee, and even as a medical consultant, we’re usually expected to contribute heavily towards the acute medical take. After the initial two, three or possibly four years of learning how to manage the take, most admission shifts are service provision with little additional training benefit. The workload of physicians is too heavy to allow a stress-free or even a low stress workplace, raising cortisol levels which stimulate fat storage.

Contrast this to other specialties such as surgery and anaesthetics who do have busy moments, but their on-calls are frequently spent off site or they have the option to sleep during night shifts on site.

Nothing ever occurs in solitude and with stress of the workplace, many physicians turn to vices such as alcohol to release stress. Alcohol itself is empty calories but also has the synergistic effect of making you crave junk food and further exacerbating the vicious cycle. Lack of free time, either from exhaustion or simply the number of hours spent on site, will also make it difficult to prepare healthy meals.

Lastly there is certainly a “fitness culture” amongst some specialties that is probably void in medicine. With the pressure to fit in, sometimes it can be seen as slightly odd for a physician to be so interested in exercise and fitness when few around them are. The risk of being seen as a misfit can even make some physicians avoid exercise, or acts as a convenient excuse not to do so.
