A Respiratory Registrar’s Thoughts on Out Of Programme Research (OOPR)

Medical training from the beginning of medical school to the completion of higher specialty training takes a minimum of 14 years calculated by 5 university, 2 foundation, 2 – 3 core or internal medical training and 4 – 5 higher specialty training years. Even then education does not stop once we become consultants thus the coined phrase lifelong learning.

Is out of programme research (OOPR) mandatory for choice of consultant post?

The short answer is no. I’d say that aside from being able to get along with your colleagues, timing of vacancies is the most important deciding factor whether we achieve a consultant post or not. The advantage of OOPR experience leading to a higher postgraduate MD or PhD degree enters when we compare two candidates completing their higher specialist training simultaneously and competing for the same spot. The same could be said for teaching qualifications or additional procedural skills out with the standard curriculum.

Can’t we be great physicians without research experience?

I’ve met some impressive doctors who didn’t take time out of their training path to dedicate towards research but the clear question is whether or not they would’ve become even more impressive if they did. My answer here will undoubtedly be biased but having been a competent registrar prior to OOPR, I feel even more competent now after these two years of research. Whether or not that’s a false sense of security from being able to focus on one single disease entity in significant detail or it’s simply a case of maturing more as a doctor from having two more years of experience, I’m not sure.

Can we perform research without taking time out of training?

Theoretically, research can always be done alongside clinical work as a registrar and in hindsight there was nothing stopping me from collecting data, performing statistical analysis and writing manuscripts that have led to 40+ publications over the past two years. However, medical rotas are quite hefty these days and it’s not unusual for us to have to do one set of night shifts every month – I have yet to come across a pre-OOPR medical registrar able to balance clinical and academic commitments. Furthermore, there really is no substitute for an experienced mentor who can guide you through the proper process of data collection, statistics and medical writing. Clinical mentors are simply too busy with clinical work and are probably not interested in academia enough to do this. Both research protected time and an academic supervisor can really only be achieved by doing a postgraduate degree.

Of course, some research work such as prospective trials cannot be done with a focused mind alongside clinical work (covid is an exception) although for trainees who absolutely do not want to pursue research, this book is useful for learning about key research principles.
